I understand and certify that Lori and Bob Hollander or any associate
of Hollander Counseling Associates LLC will provide me with information
about communication techniques only. Although Lori and Bob Hollander
or any associate of Hollander Counseling Associates LLC are psychotherapists,
the information provided by Lori and Bob Hollander or any associate
of Hollander Counseling Associates LLC is for educational purposes
only and to provide educational information. It is not intended
to be professional psychotherapy or counseling in any form or fashion,
nor is the information intended to convey specific and individual
advice. I understand and certify that Lori and Bob Hollander or
any associate of Hollander Counseling Associates LLC are not treating
any physical, psychiatric or psychological condition that I may
have. I am totally responsible if I choose to implement any techniques
that I learn from Lori and Bob Hollander or any associate of Hollander
Counseling Associates LLC and realize I need not accept any educational
information that Lori and Bob Hollander or any associate of Hollander
Counseling Associates LLC presents. I will not discontinue any psychiatric,
psychological or physical treatment that I am currently receiving
as a result of the information and instruction provided by Lori
and Bob Hollander or any associate of Hollander Counseling Associates
LLC.
I agree to consult my psychotherapist or physician before acting
upon information or implementing any techniques that Lori and Bob
Hollander or any associate of Hollander Counseling Associates LLC
may provide. If the information presented is used without consultation
and approval of a physician or psychotherapist, the individual will
be diagnosing for him or herself. No responsibility is assumed by
Lori and Bob Hollander or any associate of Hollander Counseling
Associates LLC for the use of the information contained herein in
lieu of a physician's or psychotherapist’s services. The information
presented herein by Lori and Bob Hollander or any associate of Hollander
Counseling Associates LLC is not intended to diagnose, treat, cure
or prevent any disease, psychiatric or psychological condition.
